We greatly appreciate your interest in hosting a donation drive for Foodlink. Upon receipt of this form, we will confirm your drive by sending you an email that will include your Donor Code. Please label all boxes and containers of donations with your Donor Code. This is important for donation tracking and will allow us to accurately recognize your generosity.

Foodlink can provide pick-up, if necessary, for any donations in excess of twently (20) medium sized moving boxes (approximately 18" x 16" x 18") with one week's advance notice. For donations of less than 20 boxes, you must schedule a time to drop off your donations at Foodlink.

As a general rule, donation drives hosted on behalf of Foodlink should:
- Adhere to Foodlink branding identity and standards.
- Have a positive impact on Foodlink's reputation and be algined with Foodlink's mission and values.
- Generate sufficient benefit to justify the commitment of requested Foodlink resources.
